You are paying for clicks. Somebody taps the call button, your phone rings, and reception is with a patient. It rings out. That person calls the next clinic on the list and you never find out they existed.
Google will not help you here. In India, Google cannot give you a forwarding number, which means its "calls from ads" figure counts taps on a button, not conversations. A twelve-second misdial and a booked appointment look identical in your reports.
